Good day,

I have sss online account but unfortunately, I can't open it because i forgot my userID and password also my email address that i used to registering in my sss account.

May i request that my email address is replaced with [cuetoroseann1227@gmail.com] which is my active email address.

Here are my details relevant to this purpose:

Name: Cueto, Rose Ann Joy Besmar

Date of Birth: December 27, 2001

SSS/CRN number: 35-0346104-6

I have also two copies of my valid ID to proof of my identity.
