Good day Sir/Ma'am,
Tanung ko lang po kung anu pa po ang pwede ko pong supporting docs para makuha ko po yung maternity benefits ko po.wala po kasing death cert c baby ko po kasi po wala po kasi sya s min age/weight na ini issuehan ng death cert.my baby is only 480g/6mos.may medical cert naman pong binigay c dra na naka indicate po dun na wla po tlgang death cert yung mga ganung case.Hoping for your immediate response on this matter.Thank you po
