Hi, I would like to request assistance regarding my sickness notification being rejected. Was being asked to re file and submit a certified true copy of the operation procedure. I undergone laparoscopic surgery. I already submitted all documents needed and it was my employer who submitted the forms and documents. I paid for the said documents from the hospital. Was being asked to refile but I no longer have the luxury of time to get or complete the documents again sue to the fact that I am also working. I was expecting that everything is good since I know I have done my part. Thank you
