Good Day!

I have a SSS online account but i forgot my userID and password unfortunately.I also forgot my email account that i used in registering in My SSS.

May i request that my email address be replaced with jherwin.tanteo@yahoo.com which is my active email as of my date.

Here are my details relevant to this purpose:

Date of Birth: November 02,1984 

SSS/CRN number:0033-8385582-7
 
I've also attached two copies of my valid ID as proof of my identity.
