I have a SSS online account but i forgot my userID and password unfortunately. I also forgot my email account that i used in registering in My. 

SSS. May i request that my email address be replaced with [fernandobien1964@gmail.com] which is my active email as of date. 

Here are my details relevant to this purpose: Name: Bien, Fernando Escalada

 Date of birth: September 25, 1964 
SSS/CRN number: 03-8289261-9 

I've also attached two copies of my valid ID as proof of my identity.

Your right to request a review
