Good day,

May nga katanungan lang po ako.

1. Pwede po ba gamitin ang approved SSS Sickness Form SIC-01252 sa pag-apply ng EC Sickness?

2. Kailangan ko pa rin po ba magpasa ng SSS Reimbursement SIC-01253 kahit naka home quarantine lang po ako at kung kahit wala na po ako balak ipa-reimburse yung mga nagasto ko sa gamot?

3. Anu-anung form name ang kailangan pa po ipasa para maka-avail ng EC Voucher for Covid Case?

Isa po akong Covid victim at nais mag-apply ng Cash Assistance ng ECC.
