Hello! I hope all is we
I am Charmane Tamundong, a PWD with SSS#
3405736264. 
I would like to seek for your help to change my SSS membership status to SELF EMPLOYED.
I personally submitted my filled out Membership Data Change Request Form to SSS Santa Rosa branch on 08/09/22. They said that they will forward my request to San Pablo Branch as they are the one who will process it. They said they will process it within 3 weekss. I went to Santa Rosa Branch again on 09/06/22 to follow up but until now my membership is not yet updated. Hoping that you can help me w/ this. Thank you!

Please find copy of my filled out Membership Data  Request form w/ received stamp by SSS Santa Rosa.

Please help w/ this. It seems like that SSS is not processing my request. It is very difficult for me to follow up everytime to their office personally as I am a PWD.
