Hi, Good day!

I hope this email finds you well. I am writing you this with a sole purpose of reconsidering my EC Claims for COVID-19. Unfortunately, due to contraction of this known disease cost me to loss my Job and the eligibility to take my Board Exam or LET last September 26, 2021. It has been a tough circumstances that made me financially unstable. Supposedly, the claim should be processed but since I'm no longer part of the company then, I have to do the request myself; unknowingly missing to fill out the part 3 of my Sickness Notification. Hoping you can understand and help me out.
