Hi Ma’am /Sir , 

 I would like to ask  regarding the personal information of my uncle that his record doesn’t match in your record .  

Here is the email  from SSS Web Registration 

We regret to inform you that your registration in the SSS Website was unsuccessful due to the following reason/s:

* Your Name and/ or date of birth does not match your SSS records. 

We went at the Tanay Branch  the officer told us that their record is okay but until now we received the same email on  mismatch of information . 

 I hope that you will give attention in this matter .Thank you !
