Hello Good day Ma'am/Sir

 I'm Angelo  Baga Uayan,I would like to request a change number an email of my SSS cause I lost my phone a year ago an I need to open my account to continue paying my loan in SSS,I hope you can help me..I need it badly for my job an my family I forget my password too for a long time of not using it, hoping for your kind consideration an quick response...
