Hi,
For EC Covid-19 Loss of Income and Reimbursement benefit, you said:
Entries in the logbook shall be made within five (5) days from notice of knowledge of the occurrence of the contingency. Within five (5) days after entry in the logbook, the employer shall report to the SSS those contingencies it deems to be work-connected.

My question is:
1.) How do we report to SSS? Do we send email? Do we have a report format? Do we call them or visit the SSS branch?
2.) What is the step by step process in reporting as there are no available write up for this that we can follow.
