Hello, i have made an enrollment application form to your institution but due to some certain circumstances, i currently do not remember what the username and password of my account is. Would it be possible to send said information through either if not both my email and facebook account. And if not, how would i go about recovering such information.

While our aim is to provide information whenever possible, in this instance, your request is part of the frontline service of the University. You may send an email to the office concerned, send an email the University Information Technology Center (UITC) through email address uitc@tup.edu.ph or call at trunkline number 53013001 and connect to your concerned office.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Dr. Reynaldo P. Ramos, TUP President through email address tup@tup.edu.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
