Good day!

I am Catherine Comsti, a bona-fide 5th year student taking BS in Architecture in Polytechnic University of the Philippines. I would like to request the following data and documents:

1. Yearly Statistical Data of Geriatric patients or elderly patients. (at least for the past 5 years.)
2. Guidelines in designing a facility for Geriatric Medicine.
3. Current Organizational Chart of Philippine General Hospital

This will be very beneficial for my undergraduate thesis research.

Thank you for your request dated Nov 09, 2020 08:58:22 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Statistical Data of Geriatric Patients.

Your FOI request is approved. I enclose a copy of some of the information you requested.

Please note that we are only able to provide some of the information you have requested for as we do not have yet the "Guidelines in designing a facility for Geriatric Medicine" (item no. 2) as the master planning of PGH, according to the office of the Deputy Director for Administration, is still ongoing.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Vice Chancellor Michael L. Tee at foirc.upmanila@up.edu.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
