Greetings!

Thank you for your request dated Sep 05, 2023 10:12:45 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Floor plans of UP-PGH.

Concerning your request for a high resolution copy of the floor plans of the PGH for your architectural thesis, may we request you to seek endorsement first from the UP Manila Research Ethics Board (UPM REB)? You may access the UPM REB’s website through this link: reb.upm.edu.ph.  Subsequently, you are requested to register your research with the Research Grants Administration Office (RGAO). Once appropriate clearances have been obtained, the PGH Expanded Hospital Research Office (EHRO) will issue a permit for the request to proceed.

For clarification and other inquiries pertaining to the conduct of research in PGH, the EHRO can be reached at telephone number 8554-8400 local 2064. Its office is located on the second floor of the UP PGH.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Dr. Michael L. Tee at foirc.upmanila@up.edu.ph.. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
