Dear Zach Balasta,

I refer to my letter of Apr 12, 2023 03:16:31 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Scheduling Policy - Philippine General Hospital.

As advised by the UPM-PGH, to proceed with this FOI request, please be guided by the following:
1) The requester shall seek endorsement first from the UP Manila Research Ethics Board (UPMREB) prior to the approval of this request. There are forms to be filled out and instructions are given in the UPMREB website (reb.upm.edu.ph).
2) Subsequently, the requester shall register with the Research Grants Administration Office (RGAO) via online.
3) Once clearance has been obtained, the PGH Expanded Hospital Research Office (EHRO) will issue a permit for the request to proceed and be acted upon by the PGH HRDD.
4) For clarification and other inquiries pertaining to the conduct of research in PGH, the EHRO can be reached at telephone number 8554-8400 local 2064. Its office is located on the second floor of UP PGH.


Once you have the permit you may send it to using the reply function on the eFOI portal at https://pcoomaster11222017130000-dot-efoi-ph.appspot.com/requests/aglzfmVmb2ktcGhyHQsSB0NvbnRlbnQiEFVQTS0xODc5MDAwNTI1NDUM, for request with ticket number #UPM-187900052545, to respond.

I refer to my letter of Apr 19, 2023 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Scheduling Policy - Philippine General Hospital. In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.

As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Jul 12, 2023.

You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
