I would like to ask for the following:

1. Site Plans of existing residential care facilities/group home facilities for PWDs
2. Floor Plans of existing residential care facilities/group home facilities;
3. List of programs offered in the facility (detailed if possible);
4. Daily schedule and activities of PWDs clients and the caregivers
5. number of rooms and beds
6. contact persons for each facility
