Hi! I would like to request for a list of facilities that provides continuum care for PWDs. I am currently undertaking an architectural thesis that aims to address the challenges faced by PWDs by establishing a continuum care facility. I would also like to know the contact persons whom I can reach for each facility. Thanks in advance.
