Silverio 09:28 AM, Nov 20, 2023

am writing to request a disability certification for my mother, SHIRLEY ESCOTO ATO, Age 53, about an unfortunate incident that occurred in our neighborhood.

There was an incident involving the discharge of a firearm near our residence. As a result of this incident, my mother experienced immediate and severe hearing impairment and had cataract eyes already moments, she lost her hearing entirely, which gradually started to return after a short duration. Regrettably, her hearing did not fully recover, and subsequent examinations confirmed the presence of a ruptured eardrum.
