I trust this message finds you well. My name is Vincent, and I am seeking assistance in transferring my PWD card from Mindoro to Quezon City. I have been residing back and forth between Quezon City and Palawan and recently relocated permanently to Quezon City. 

Unfortunately, I no longer have contacts in Mindoro to facilitate the cancellation of my current PWD card.

As advised by the PWDAO in Quezon City, it is necessary to cancel my existing PWD card in Mindoro before I can request a new booklet here. 

I am unable to go there due to my health condition. Can I do it online? please.

For additional information, my PWD Card was issued on October 14, 2021 and to expire on the same day this year.

I can provide my PWD ID # upon request.
