Good day! This is to request from your good office a Certificate of Confirmation that I was belong to a Capiznon Tribe/Clan. Both of my parents and ancestors are born and lived in Sapi-an Agsur, Capiz.

This request will be use for my application in Bureau of Jail Management and Penology (BJMP) as a Height Waiver.

I am hoping for your kind consideration regarding this matter.

Response to your request

While our aim is to provide information whenever possible, in this instance the NCIP Central Office is no longer processing such certificate but in the Regional Office . However, you may wish to contact  Dir. Ana Burgos Regional Director Region 6/7 at region67@ncip.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at <insert link here>.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Dir Ana Burgos at region67@ncip.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
