TUMACA 03:27 PM, Feb 03, 2023

Good day sir/mam!!

I am Richell Tumaca, a government employee.I am writing a request in behalf of my husband Regie who is so interested to join the jail bureau yet needs to secure for an age waiver certificate for his jail officer 1 application.

He went to San Jose, Romblon just last month to process the age waiver certificate from NCIP but failed to secure same because of some errors noted in the birth certificate of his mother.

In this connection, may I request from your end that the defeciencies found in his mother's birth certificate be considered so he may able to secure age waiver.

Dear RICHELL,

Greetings!

Thank you for your request dated Feb 03, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request

You asked for Age Waiver for my Husband.

Response to your request

While our aim is to provide information whenever possible, in this instance the NCIP Central Office is no longer processing such certification but in the Regional Office and they are the one validating the documents required and since there are discrepancies in your mother in law's birth certificate the best agency to correct such error is the Philippine Statistics Authority. You may addressed your other concern to Regional Director Marie Grace Pascua of Region 4B at r4b@ncip.gov.ph or resubmit your request addressed to the correct agency on the eFOI portal at <insert link here>.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Director Marie Grace Pascua at r4b@ncip.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
