Greetings of Peace! I would like to request a Certificate of Tribal Membership on Muslim Filipinos. I was born and raised in Isulan, Sultan Kudarat and I believe this will help and serve as basis on the approval of my request.The said certificate is a requirement to waive my height as I will be applying to be a part of the Philippine National Police. The approval of my request will be much appreciated. Thank you!

Response to your request

While our aim is to provide information whenever possible, in this instance this Office cannot issue a Certificate of Tribal Membership without your application submitted to this Office.

Your right to request a review

Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
