I kindly request data on number of patients who had dialysis for the 1st time per health facility (w/ health facility's location - city or municipality) from 2015 to 2022.

I refer to my letter of Oct 30, 2023 04:58:53 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 1st time dialysis patients from 2015-2022.

Ill clarify first with our Public Health Unit what data we can provide with regards to your FOI.

Please note that we are only able to provide some of the information you have requested. I enclose a copy of the dataset in jpg of the information you requested.  Ill try to send via email other data that we might have.

Your right to request a review

Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
