You asked for Data gathering for undergraduate thesis.

Response to Request

Your FOI request is approved. I have coordinated with the Disease Prevention Control Bureau.  Please send your request to Ana Marie Selina G. Garfin, MDMM, Director IV of the Disease Prevention Control Bureau through their email address dpcvstrategies@doh.gov.ph.  The Disease Prevention Control Bureau will provide you the data of other hospitals not only the data of the National Kidney and Transplant Institute.
