Please provide raw data without PII of cancer patients in your hospital on the following on a monthly basis:

1. Vaccination status/date of vaccination for covid-19.
2. Age.
3. Kind and stage of cancer. 
4. Onset of cancer.

You asked for Information on cancer patients.

Response to Request

Your FOI request is approved. I enclose a copy of some of the information you requested.

Please note that we are only able to provide some of the information you have requested that is available in our hospital for cancer patients.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <josephine.hubilla@nkti.gov.ph>. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
