Im Jan Erck A. Labasan grade 12 student at San Francisco Highschool I would like to have a permission to conduct a research with patients with kidney failure. Our research title is (Correlational Study of Diet and Lifestyle Among Patients with Kidney Failure: Impact on Disease Progression) to approve this research title our teacher at PR 2 told that we must find out first if its possible to conduct this research in this place. Am Hoping that you will give as permission to conduct this research. (Our Updated Id is not yet given)

Hi sorry for late response, So the specific study we want to conduct is the correlational of diet and lifestyle among patients with chronic kidney disease. We are going to ask some survey or questionnaires in patients what are they usually eating or diets and the lifestyle if they're  smoking or they sleeping schedule. We are still thinking what are the questions we would like to ask to the patients but we are only asking those who are willing to be ask we are not going to force anyone and We would like to see the medical record of the patients that we interview to see the progression of the chronic kidney disease if by any chance that we cant get the medical record we would like to know if there is any methods  that we can see the progression of chronic kidney disease  to the patients that we interview. We are hoping for approval of FOI . As a grade 12 student of San Francisco High School we will do our best to conduct this research in NKTI hospital. (Team D: Dream Team Researchers)
