Good day, may request for the following:

1. report on the prevalence of CKD - last 5 yrs up to the latest
2. report on the number/prevalence of CKD patients/ESRD pts on hemodialysis - last 5 yrs up to the latest, including the leading causes
3. mortality data on ESRD on dialysis from last 5 yrs up to the present

While our aim is to provide information whenever possible, in this instance our Public Health Unit Office does not have the information you have requested as our available information is up to year 2016 only. 

You may post a new FOI reflecting date parameters that we can provide e.g. 2011 to 2016.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the undersign at francus.angeles@nkti.gov.ph.
