You asked for Number of hemodialysis patient in Quezon city.

Your FOI request is approved. 

for item #2... I enclosed a PDF copy of a list of Free Standing DCs in the Philippines. From the list you can check the NCR listing Dialysis Centers in QC at Page 4.

For item #1... Is it the number of QC residents undergoing dialysis at NKTI only?  or in the entire NCR?
