Hi. I would like to request a copy of my certificate in COSH training (SO1 and SO2) here are the details

1. It was held at "The Ouan's Farm Resort Diversion Road, Kanlurang Mayao, Lucena City, Quezon on April 2-6, 2019

Dear Ms. Gladys Buñales,

This refers to your request dated Jan 26, 2024 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for a copy of certificate in COSH training.

1. photo of the damage certificate, or
2. in case of loss certificate, Notarized Affidavit of Loss
