Greetings!

Thank you for your request dated Sep 28, 2020 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request for Certificate of unvoluntary seperation is taken from employment agency or employer and in addition for the ofws from the POLO/POEA or the Philippine Embassy.  This is part of the ofw's statement when they are repatriated as this document is attached to the referral to the OWWA/LGUs/DSWD.


While our aim is to provide information whenever possible, in this instance we would need for you to submit your request to appropriate offices.
