You asked for RESOLUTION No. 1409 s 2018.

Response to Request

Your FOI request is approved. The certified true photocopies of CESB Resolution Number 1409 dated 12 July 2018 are now ready for pick-up at the CESB office located at Number 3 Marcelino Street, Isidora Hills, Diliman, Quezon City. Please prepare PhP600.00 for the authentication fee. You may also opt to deposit your payment to CESB's Land Bank of the Philippines account (Account Name: Career Executive Service Board; Account Number: 0622-1022-34). Kindly furnish us through email (rtuando@cesboard.gov.ph) a copy of the deposit slip indicating the name of depositor and the branch where deposited for the issuance of the official receipt. During pick up, please look for Mr. Alexander A. Bandala of the CESB’s Policy, Planning, and Legal Division (PPLD). He can be reached at mobile number 0927 677 7159 or through email at abandala@cesboard.gov.ph.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to CESB Executive Director Maria Marcy Cosare-Ballesteros with principal address indicated above. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
