Good day Ma’am and Sir!

I hope this letter finds you in good health and high spirits. My name is Alenn Pasignajen, and I am currently a third-year student enrolled in the Bachelor of Secondary Education Major in English program at Trinidad Municipal College in Trinidad Bohol.

I am writing to inquire about the process of obtaining my Form 137 and 138 for Taiwan Scholarship requirements purposes.

Greetings!

Thank you for your request dated Jan 22, 2024, under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for FORM 137 and 138.

Response to your request

While our aim is to provide information whenever possible, your request for a copy of Form 137 and 138 could be best addressed by your alma mater since they have the original copy of your requested file. Furthermore, the release of high school transcripts is not under CHED's jurisdiction.

Your right to request a review:

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to chairperson@ched.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
