I am Lea D. Bejar. I am writing this letter to request for my National Service Training Program (NSTP) Serial Number from my former school, St. Mary’s College of Borongan. I need the said certificate for my application for graduation in University of Southern Philippines Foundation the soonest possible time, as well as a requirement for my enrollment in the upcoming semester, S.Y. 2023-2024.
I took my NSTP class in SMCB during my freshman year, A.Y. 2018-2019.

Thank you for your request dated  Nov 13, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for NSTP Serial Number.

Please be advised that per Implementing Rules and Regulations of the Republic Act No. 9163 or “National Service Training Program (NSTP) Act of 2001”, the Higher Education Institution (or your school) is responsible for securing the NSTP serial number upon submission of the NSTP Enrollment List and request letter, duly signed by the NSTP Director and/or registrar.

Given these, may we advise you to contact your school registrar where you finished your NSTP to help you on this matter.
