Good day!

I am Jeremy Velasco Gayta, graduate of Bs Mechanical Engineering in Southern Luzon State University in Lucban, Quezon on March 2014.
I will take Master Plumber Board Examination. The Philippine Regulatory Commission (PRC) is requiring me a Transcript of Records (TOR) with Special Order Number. 
I would like to request my Special Order Number. Or is it already included in my TOR once i request a new copy?
