Hi!
 
I’d like to request for the following:
 
1. Transcript of Records from Montessori Professional College school years 2017-2019.
 
Montessori Professional College (President: Mr. Danilo M. Dy) has permanently closed and I’m hoping you can provide me a copy of my Transcript of Records and Diploma. I’d also appreciate it if you can provide documentation showing the institution ceased operation.

It should be form 137 not TOR, I am very sorry. I am requesting a document of form 137 and diploma. Thank you!

Response to your request

While our aim is to provide information whenever possible, your request for a copy of your Form 137and diploma could be best addressed by the Department of Education (DepEd) since high school credentials are under their jurisdiction. 

Also, please be informed that CHED FOI only caters to inquiries and requests that are related to college or higher education matters.

Your right to request a review:

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to chairperson@ched.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
