To whom it my concern, 

I would like to change my Birthdate on my vaccine card. I've written the wrong Birthdate when I was signing up for my 1st dose. Please change it from April 16, 2001 to April 30, 2001. I might run into some problems in the future with any of my government documents.

Please visit the VaxCerPH portal which is the proper authority to take action on your request re: rectification of birthdate appearing in your vaccine card.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to op@clsu.edu.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
