1. Hello Good afternoon. I would like to request for my ROTC serial Number. 
2. I started my College degree in Central Luzon State University Year 2004 up to 2009 with a Bachelor's Degree in Biology. 
3. Within those years I had a 2 Semester or 1 year course of ROTC from 2004 to 2005 belonging to the Military Police Unit. 
4. I was Hoping to retrieved my Serial Number and apply/change my status from a regular civilian to Army Reservist.
