You asked for Procedure and requirements for Undergraduate CAV.

Response to Request

You can send your request online to the official email for document request at registrarmain.request@cvsu.edu.ph .  Upon verification, the assigned registrar will send a link for you to accomplish the Google Form.  Upon confirmation, the assigned registrar will acknowledge your request thru e-mail together with the list of requirements to bring and fees to be paid.  For the issuance of CAV, you will be informed by the assigned registrar of the scheduled date for authentication of documents following the health protocols implemented by the University.
