To whom it may concern,

Good day! I am a former student from Cavite State University.  This school year, I transferred to Bulacan State University. I took my Nstp around June 2015 - April 2016 from CvSU. As per IRR of NSTP Section 4.a.2, I am not required to take the said subject again. But my professor in the subject needed my NSTP Serial Number for assurance. Thus, hoping that you could give me a printable record/ document of my serial number. I hope you could get back to me as soon as possible.  Thank you and God Bless!

Dear Krizza May Metin,

I refer to my letter of Jan 25, 2021 04:29:13 PM; about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for NSTP SERIAL NUMBER.

Our response to your request is due on Feb 15, 2021 04:29:13 PM. Unfortunately, it is taking longer than expected to deal with your request because as per Mr. Adonis Penales, CvSU ROTC Coordinator, the ROTC Serial Number supposedly assigned for batch AY 2015-2016 of Cavite was inadvertently sent to Batangas. This information was given to him when he went to Camp Riego de Dios last week to facilitate your request. He then requested for the Serial Number of the said batch and was advised that the said Serial Numbers will available by March.

I apologize for this delay but have applied for an extension so we could work further on your request. We hope to be able to send you a response on or before Mar 15, 2021 04:29:13 PM.

Please be informed that the ROTC serial numbers being requested by Camp Riego de Dios for the province of Cavite are now available, however, the NSTP coordinator is still awaiting for the formal transmittal of the serial numbers assigned to CvSU.
