Good day, i'm Edmon B. De ocampo Architecture Graduate in CVSU main campus. i would like to request a copy of my diploma and transcript of record. i graduated november 2010

Greetings!

Thank you for your request dated Jul 17, 2023 11:28:36 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for DIPLOMA & TOR.

Response to Request

The following is the procedure to follow relative to your request for diploma and transcript of records:

1. Make an online request to  the official email for document request of the Registrar's Office  which is   registrarmain.request@cvsu.edu.ph
2. Upon verification, the assigned registrar will send you a link for you to accomplish the Google Form. 
3. Upon confirmation of your request, the assigned registrar will send an acknowledgement email to you and will make a schedule for the release of the documents you have requested (TOR and Diploma).
4. For the Issuance of Original Copy of Documents, you will be informed through e-mail of the status of request and the fees to be paid upon claiming the documents.  You will also be informed of the schedule of release of the documents following the health protocols being implemented by the University.
