Hi! I would like to request another vaccine card because mine got lost. I have picture of my vaccine card but I took picture in my first dose I don’t Have picture in my 2nd dose however I remember when and where I got vaccinated. I got my first dose at July 30, 2021 in Magallanes Elementary School, Davao City and my second dose at August 2021 at the same place under the name Alexandria Ochida Delima. I tried searching for my record in VaxCertPh, it says that I donthave any record. I hope my request gets fulfilled for I will be using it
