Hi good day! I would like to follow up my Vax Cert request with my:

1. Reference ID: 3760475

My request for Vaxcert is rejected and i have been trying to access my vaccination since July 8 2022. However your system always declines it and always tells me that there's no vaccination record found. I have already submitted my copy of vaccination card and my Postal ID in your system.

I hope you will consider my request. Thank you

While our aim is to provide information whenever possible, in this instance this concern can be best addressed by the Department of Health.

VaxCertPH is a portal and mobile app developed by the DICT for the Department of Health (DOH) for the issuance of digital vaccination certificates for vaccinated individuals in the country. Please be informed that the DICT does not handle management of personal information of vaccinees in the portal.

Your right to request a review
