Hi! I would like to follow up my Vax Cert PH Request with: 

1. Reference ID: 101686

To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last month, June 2022. 

However, your system always declines it and always tells me that there is no vaccination record found. I have already submitted a copy of my vaccination card and my valid ID in your system, with a reference ID number of 101686.

I am really hoping you can help me.

While our aim is to provide information whenever possible, in this instance this concern can be best addressed by the Department of Health.

VaxCertPH is a portal and mobile app developed by the DICT for the Department of Health (DOH) for the issuance of digital vaccination certificates for vaccinated individuals in the country. Please be informed that the DICT does not handle management of personal information of vaccinees in the portal.
