Greetings! I would like to request to update my vaccination record. I already contacted the LGUs but they cannot update my record because the system is down. I also contacted the VaxCertPH Support and replied that they unable to access the Digital Vaccination Certificate (DVC) portal and my information was already forwarded to your good office to update my record. Here are my VaxCertPH Reference ID request #s: 3987320, 3987321, 3987323.

My agency/employer requires this certificate in order to book my flight ticket. Thank you for your consideration and attention to this matter.

Response to your request

While our aim is to provide information whenever possible, in this instance this concern can be best addressed by the Department of Health.

VaxCertPH is a portal and mobile app developed by the DICT for the Department of Health (DOH) for the issuance of digital vaccination certificates for vaccinated individuals in the country. Please be informed that the DICT does not handle management of personal information of vaccinees in the portal.

Your right to request a review
