Thank you for your request dated Jan 12, 2024 01:43:04 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for dole Certificate of Unemployment.

Response to Request

This is to inform you of the process and requirements by which to apply for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION from your employer.  An employee who was involuntarily separated by their employer may file their application for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION at the following through e-mail: 

1. For Local Workers - DOLE Field/Provincial Office (for local workers) where the company or employer is located or where the employee resides; 

2. For Overseas Filipino workers - Philippine Overseas Labor Office- POLO where their employer operates or at the DOLE Field or Provincial Office where the OFW resides.

The requirements to substantiate the issuance of the certificate of involuntary separation are as follows: 

a.	at least one (1) valid I.D. 
b.	copy of the Notice of Termination issued by the employer, in the absence of Notice of Termination, an applicant may submit a duly notarized Affidavit of Termination of Employment 

Within one (1) day upon receipt and verification of application with complete documentary requirements, the DOLE Field or Provincial Office or Philippine Overseas Labor Office shall issue the certification.

Your right to request a review
