Good day Sir/ Maam!

      May I please request from your good office a copy of a certification of involuntary separation as a requirement for my filing of unemployment benefits? I tried to search for the e-services website for region 5 - Camarines Sur but I couldn't find any. I also send a request to the ff. email address:

I refer to your letter of Sep 03, 2022 10:34:55 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for CERTIFICATE OF INVOLUNTARY SEPARATION.

2. Photocopy of at least one (1) valid Government Issued ID
3. Notice of Termination/Duly Notarized Affidavit of Termination
