Good day I would like to make a request regarding the affidavit of guardianship of dost, one of the needed requirements for dost and I make my aunt my guardian but I can't proceed with the application since I don't have the needed form. Fo I still need to upload an affidavit even if I'm not a minor anymore?

Hoping for a response soon since there is only a week before the dost close their application... Thank you
