Good day, I am an incoming 1st year college student applying for DOST-SEI Scholarship. I have encountered in the last part of my application that I need to upload an affidavit of guardianship. I have declared my aunt as my legal guardian. How can I get my affidavit of guardianship form? 

I hope this urgent request finds you well, since there's only a few days before the application ends.
