Dear MS. RANI OLAVIDES,

This refers to your eFOI request received on 05 June 2023 via the eFOI portal of the Home Development Mutual Fund (Fund) wherein you inquired regarding the registration of your household helper with the Fund using the Unified Kasambahay Registration Form.

According to you, you visited one of our Branches to register your household helper, but the Branch allegedly refused to receive the said registration Form. Hence, you are now asking where to register using the said Form.

We have endorsed your concern to the operating unit concerned (OUC) – the Member Services Support Unit (MSSU). We were informed by said OUC that you can visit any Pag-IBIG Branch for the registration of your household helper and need to submit the following accomplished documents:

For the Household Employer

•	Household Employer Unified Registration Form (PPS-HEUR1)
•	Household Employment Unified Report Form (PPS-HEUR2)
•	Specimen Signature Form (HQP-PFF-003)

For the Kasambahay (household helper)

•	Kasambahay Unified Registration Form (PPS-KURF)

The above listed forms may be downloaded thru our website https://www.pagibigfund.gov.ph/forms_provident.html.

Further, the said household helper may register thru any of the following modes:

•	Online membership registration thru Pag-IBIG Fund website at www.pagibigfund.gov.ph
•	Any designated service desk center of PhilHealth and SSS.
•	If registered online with Pag-IBIG Fund, the supporting documents shall be submitted to concerned agencies only upon availment of benefits.

Note: If filed/submitted thru the authorized representative of the household employer, the following documents shall also be presented:

•	Letter of Authorization
•	Valid ID of the household employer
•	Valid ID of the authorized representative
