Dear Ms. Orbase,

Greetings from the Philippine Crop Insurance Corporation (PCIC)!

Thank you for your request dated August 13, 2021 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request:

You have requested for a list of registered cooperatives in Region 5, Philippines, dated January 1, 2000 to December 31, 2000.

Response to your request:

While our aim is to provide information whenever possible, in this instance, the PCIC does not have the information you have requested. However, you may wish to contact the Cooperative Development Authority who may be able to assist you with your request.

Your right to request a review:

If you are not satisfied with this response to your FOI request, an appeal may be filed by sending an email to admin@pcic.gov.ph. Your appeal should explain the reason for dissatisfaction, and should be made within fifteen (15) calendar days from receipt of this letter. The PCIC will complete the review and notify you within thirty (30) calendar days from receipt of the appeal.

If you are not satisfied with result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
